MACC has dental and vision insurance coverage available to eligible employees. As of January 1, 2008, our dental coverage is with Guardian Life Insurance Co. Preventive services such as exams and cleanings are covered at 100% with no deductible. Basic services are covered at 80%. Major services are covered at 50% after a 12 month waiting period. A $50 annual per person deductible (maximum of $150 per family) applies to basic and major services. Vision coverage is with Guardian and utilizes the VSP network. This plan covers exams and glasses or contact lenses. It is a PPO plan, with level of coverage determined by use of a network provider or non-network provider.
